1. ONLINE

   • Fill out the form below with your information, and it will be securely sent to the deparment​​

2. IN-PERSON

   • Visit the Jackson Police Department during administrative office hours for a written submission form.​

   • Our hours are:

     • Monday thru Friday 9A.M.-4P.M.​

3. MAIL

   • Download the PDF below, complete the submission form on the last panel, and mail to the address listed on the form.
